decisions.current_stage_index + instantiate_stage() in same transaction as stage completion; terminal only after last stage (currently ANY step completion terminalizes the whole decision - 2-stage policies approve after stage 1). Tests: 2-stage flow incl. stage-2 task+mail rows; stage-1 reject short-circuits. Phase 2.
