UNIQUE(tenant_id, idempotency_key) migration (current index non-unique) + INSERT ON CONFLICT DO NOTHING RETURNING pattern; dedupe existing dupes in migration. Tests: same key same id (AC-3); 50-way concurrent create -> exactly 1 row. Phase 2.
